From what I can tell, you buy a Custom Apparel Token from the shop for 100 moonstones and then go to the Custom Apparel shop to create one, with the token being needed to officially send it in for review.

Once your CA's been reviewed, up to 5 copies of it'll appear on the CA shop, depending on how many you chose to keep at the start.

Of course, you ALSO need a CA design prepared to be submitted.
