Unknown item in inventory

I'm not sure when this happened, because I normally sort my inventory rather than scroll all the way down, but one of the items in the Misc. category is now unknown? I'm on Chrome if that helps.

After redoing the campaign since the update, I can definitely conirm it is one of thse soulbound quest items necessary to complete the campaign in one of the packs. Not sure which one. Luckily the icon being broken doesn't break the campaign quest story line, but for the immersion it would be nice if it could be fixed for sure.
